Thank you for placing an order for Search Institute’s Developmental Relationships Survey! The information you provide on this form will be used to generate your invoice(s) and survey link(s).

Through this form, we'll ask you to provide key details about your organization, your plans for administering the survey, the survey modules and customizations you wish to include, and some information about the young people who will be completing the survey. This will allow Search Institute to create unique survey link(s) for you so that you can administer a version of the survey that meets your measurement needs.

After you submit this form the following events will occur:

  1. You'll get an email copy of this form for your records. Please review that carefully, and if you have any questions or modifications please contact
  2. You’ll receive an invoice in accordance with the information you provided.
  3. You’ll receive an email with your login information, survey links, and administration materials. These links may be inactive until your invoice has been issued.

The username you choose and a temporary password will be emailed to you when your account is approved and set up
Note: You have requested access to a specific client account but do not have an existing username and password. Account creation will be delayed until we can verify that you are authorized to access the existing client data. In most cases, this delay should not be a problem because access to the reporting system is not necessary until after you have begun collecting survey data from your youth. If you believe that a delay of up to several weeks could be a problem, you may want to consider going back a few questions and choosing to have a new client ID created for you instead. In this case you would not have access to the older data.
