Dear Student,

Your school is part of a program called College Possible that helps students get ready for college. A company called ICF is doing a study to learn if the program is working. Student surveys are an important part of our study. ICF will use them to measure how much the program is helping students. Please read the rest of this message carefully. When you are done you can decide if you want to take the survey.

What are you asking me to do? ICF is asking you to take this 20- to 30-minute online survey. The survey asks about your interests. It asks about your future goals. It also asks how you feel about school and how much support you have to be successful after school.

Are there any advantages to me for taking the survey? No. But, if you decide to take the survey it will help us figure out how to support students better.

Will you keep the information I provide private? Yes. Our research team has to follow the law. That means we have to keep your answers private. We will never write anything about your personal survey answers. The survey does not ask for your name. We only ask for your student ID number. No one on the research team can use your student ID to find out who you are.

Are there any risks to taking the survey? No. Our research team cannot tell who you are from your student ID number and we are not asking for your name. The only risk is that the survey may take some time from your school day today. We made the survey short to reduce time out of class.

Do I have to take the survey? No. You can decide if you want to take it or not. Your school will not take away any support or programs if you decide not to. You can also skip any questions you do not want to answer. You can stop taking the survey at any time.

Does my parent/guardian know about this? Yes. We sent your parent or guardian a letter telling them that we would ask you to do a survey at school. If you are reading this message it means your parent or guardian agreed you can take the survey if you want to.

What if I have questions? If there is a teacher or someone else in the room helping you take the survey, they can help you if the survey is not working.
